An important goal of the
State Health Plan is to help members with a chronic disease or disease
risk factors better manage their condition. Through NC HealthSmart,
the Plan offers eligible members* comprehensive educational resources
and access to one-on-one nurse coaching and specialty care.
Services are available to help members* manage a chronic illness
or address a sudden, catastrophic event or medical condition requiring
multiple services and intensive management. Programs are available to
members* with chronic health conditions, including:

migraines, and muscle and bone issues. If you have a hearing or speech

Health Plan website, you can log on to your online Personal Health

Access Digital Health Coaching
personal action plan with self-paced modules to manage your chronic
condition and reach your health goals. Coaching sessions are available
to help you lower blood pressure and cholesterol, manage diabetes, ease
chronic pain and much more.
Use a Tracker
Trackers allow you to compare
your numbers to the recommended ranges and provide a quick view of your
progress over time. Trackers are available to help monitor your asthma
peak flow, blood glucose, blood pressure and more!
Speak with a Health Coach
Health coaches can help you better understand your condition, answer
questions and provide you with options to better manage your health.
Health coaches are available online or by phone Monday - Friday, 8:30
a.m. – 9 p.m., and Saturday, 9 a.m. – 2 p.m. at 800-817-7044.
If you receive a phone call from a health coach, it may be
because you would benefit from the services offered. The health coach is
calling to offer you assistance with managing your health condition, to
provide any needed information about your condition and related
medications, and to assist you with questions you may want to ask your
doctor at the next visit. The health coach contacting you is a highly
trained and experienced nurse, registered dietitian or respiratory
If you wish to stop receiving telephone calls from a health
coach, you can call 800-817-7044 toll free and request to be placed on a
"Do Not Contact" list. You will still have the ability to contact a
health coach directly to ask for help with any acute or chronic
condition by calling 800-817-7044.
Services are offered to
members* with complex medical needs such as cancer or severe accident
injuries. The program focuses on early identification of a member's need
for assistance with multiple hospitalizations or a sudden catastrophic

Members eligible for NC HealthSmart services are members
whose primary health coverage is through the State Health Plan. Federal
law prohibits the Plan from using your personal health information to
discriminate against you in any way or from giving this information to
your employing agency/school or other unauthorized third party, unless
required by law.
